NarrativeAt 1:40 a.m. on September 1, 2017, a maintenance technician was repairing a broken pin on a automatic tire balancer. LOTO procedures had not been applied to the machine. The employee opened the gate which had safety electrical interlocks installed. The top chuck is spring loaded. One spring broke and the chuck fell onto their finger. Stored energy of the top chuck with the spring and gravity had not been dissipated. The employees proceeded to remove the bolts that held the guard over the drive belt. The employee reached out and touched the belt to ensure it had stopped. The belt was still moving and snagged the glove. This pulled the employees fingers into the ingoing nip point between the belt and the pulley.

NarrativeAt 3:30 a.m. on March 16, 2017, an employee was adjusting the proximity switch flag for the bead set stop. The employee was caught between the inner bead can and the equipment cabinet while he was adjusting the flag switch causing the equipment to cycle. The employee received crushing injuries to the chest and pelvic area. The employee was hospitalized and treated for his injuries.

NarrativeAt approximately 1:00 a.m. on March 14, 2017, Employee #1 was pulling two pieces of metal rectangular tubes measuring approximately 3-inch by 6-inch by 9.67 foot long and weighing approximately 175 pounds. The metal tubes were stored leaning against a wall with other materials unrestrained. He was trying to transfer the tubes from a small wheeled storage trailer to a larger one to transport them. Employee #1 was struck in the head causing a blunt force trauma. Employee #1 died of either the blunt force trauma to his head or from a possible heart attack.
