NarrativeAt 10:30 p.m. on January 16, 2018, an employee was moving rubber material "tails" from the 400 wig-wag machine to a raised pallet supported by the forks of a forklift. The employee's right leg was caught between the pallet and the scale for the wig-wag machine, fracturing it. The employee was hospitalized.

NarrativeAt 2:00 p.m. on December 27, 2017, Employee #1 was operating an autoclave. As the cycle for curing the rubber pieces in the autoclave was complete, it was determined that all of the pressure had been released in the vessel. Employee #1 proceeded to open the door. As it opened, the pressure had not fully released and the door blew open, releasing steam onto the employee and knocking him to the ground. Employee #1 suffered steam burns to his face, chest, arms, legs and back. He was hospitalized for his injuries.

NarrativeAt 7:15 a.m. on September 21, 2017, Employee #1 was operating the hole puncher machine. The machine was built in-house and had eight sections, which are located next to each other. There are seven hole punching sections on the right side of the machine and one guillotine section on the left side of the machine. The employee was using the guillotine section to cut the product to the correct length. The machine is activated by two thumb control buttons. The control buttons are located in close proximity to the machine's point of operation so that the employees are able to activate the machine with one hand while holding the product/material with the other hand. The employee was holding the product with his right hand, and when he activated the machine, one of the punches came down on his right pinky, resulting in a partial amputation of the finger.
