NarrativeAt 10:30 p.m. on January 16, 2018, an employee was moving rubber material "tails" from the 400 wig-wag machine to a raised pallet supported by the forks of a forklift. The employee's right leg was caught between the pallet and the scale for the wig-wag machine, fracturing it. The employee was hospitalized.

NarrativeAt 10:45 p.m. on May 16, 2017, an employee was making a new product using a rubber mill. The employee cleaned the rollers by removing the excess product from the rollers. While the employee was removing the product from the rollers, a coworker switched the rubber mill on. The employee's left hand got pulled into the rollers as he was cleaning the rubber mill.

NarrativeAt approximately 2:30 a.m. on February 17, 2017, an employee was removing product from a hydraulic press. When reaching into the point of operation to remove the product (rubber car mat), one of the die plates smashed the employee's fingers. The employees finger was amputated but he was not hospitalized.
