NarrativeAt 5:30 p.m. on November 5, 2017, an employee was working from Deck 2 to fix Deck 1 when it was not functioning correctly and had been standing on a step stool. The employee stepped backwards, lost his balance, and fell 7 feet to the ground. The employee suffered a fractured skull.

NarrativeAt 5:13 p.m. on October 26, 2017, an employee was changing out materials from the slitter machine and two fingers on her right hand were caught between the chuck and material. The employee amputated the tips of both fingers. Hospitalization was not required.

NarrativeAt 11:20 a.m. on October 10, 2017, an employee was adjusting rollers and replacing rubber drive bands on a running roller conveyor. The employee caught their right index finger in a nip point, between the roller and rubber band. The employee amputated his fingertip. Hospitalization was not required.
