NarrativeAt 10:15 p.m. on January 27, 2019, an employee was cleaning molds with a hand drill and a wire brush while inside a TPM Line 1 Machine. The employee was caught in the machine when it activated and the molds closed around him, crushing the employee and fractured his skull and face. The employee was transported to the San Joaquin General Hospital where he was pronounced dead.

NarrativeAt 3:00 p.m. on July 17, 2017, an employee was checking the tension on a belt while the machinery was running. The employee caught his finger between the belt and pulley and ampurated the right index fingertip.

NarrativeAt approximately 10:15 a.m. on May 24, 2017, Employee #1 was loading a roll of paper on a wrapping machine. A forklift operator was delivering a second paper roll to the wrapping machine. The forklift operator lowered the forklift's forks causing the paper roll to move forward and fall from the forks. The paper roll struck Employee #1's left leg and caused a fibula fracture. Employee #1 was transported to a medical center, where he underwent treatment on the lower leg fracture and was then hospitalized.
