NarrativeAt 7:10 a.m. on September 28, 2017, Employee #1, a machine operator, was using an Amanda Press Brake, manufacturing small parts from sheet metal. He had manufactured approximately 60 -70 parts, following the new procedures that he had been trained on in August, holding the small piece of material with the holding tools to keep his fingers away from the danger area. As he proceeded, the employee forgot to use the tool to hold the material and placed his finger too close to the upper die of the press brake. He suffered a crushing injury to his left index finger, which resulted in the loss of the tip of the finger. Employee #1 was transported to a nearby hospital, where he remained for treatment.

NarrativeAn employee was moving a forklift mounting plate from a shelf onto a pallet. The pallet was lifted on a forklift and the mounting plates were slid from a 3.5-feet-tall shelf to the pallet. The plates have a notch in them, and while the employee was guiding the second plate onto the pallet, his right index finger was amputated at the first knuckle.

NarrativeAt approximately 9:00 a.m. on September 9, 2016, an employee was operating an impact wrench to tighten nuts and bolts on equipment used for grain handling. The grain handling equipment was raised above ground and held with a nylon strap attached to a telehandler. The strap broke and the equipment fell. The employee was working below the raised load. The employee was struck and killed by falling load.
