NarrativeOn 4/13/15, at approximately 5:00 p.m., an employee was setting up a metal grinding machine. During the setup process, he attempted to remove the part that was to be worked on. The manual control was in the reverse position at the time. As he pushed the job button with his right hand, his left ring finger was caught between the part and a machine guide, amputating his fingertip.
