NarrativeAt 2:00 p.m. on October 13, 2017, Employee #1 was tightening a bolt on the weld machine. The employee put one hand on the right side of the dual controls, and his baseball cap activated the left side, causing the machine to cycle. His left hand was crushed and lacerated. Employee #1 was hospitalized for treatment.

NarrativeAt 9:15 a.m. on September 28, 2017, Employee #1 was testing the tilting mechanism on the aerial device. The employee was on top of the basket when the lift failed, causing the basket to tilt over towards the ground. The basket fell approximately three feet and fractured his left foot. He was hospitalized for seven days.
