NarrativeAt 2:00 p.m. on October 13, 2017, Employee #1 was tightening a bolt on the weld machine. The employee put one hand on the right side of the dual controls, and his baseball cap activated the left side, causing the machine to cycle. His left hand was crushed and lacerated. Employee #1 was hospitalized for treatment.

NarrativeAt 10:45 a.m. on September 14, 2017, an employee removed the guard from a mill conveyor to lube the chain while it was in operations. The employee had his right hand pulled into the sprocket by the chain. No specific injuries were identified in the initial report other than several fingers on his right hand were crushed. The employee was hospitalized.
