NarrativeAt 5:40 a.m. on October 1, 2017, an employee was working for a manufacturer of pharmaceutical preparations. The employee was assessing a newly acquired tablet press, with identification number DD-2. He was looking at a part of the machine, the compression arm assembly, to assess if it posed an irregularity and/or a hazard. His left hand was situated at the upper portion of the metal plate near the spring. A coworker, who was with him in the machine room, moved the compression wheel of the compression assembly. When the coworker moved the compression wheel, the compression arm assembly came loose. It swung toward the spring, crushing the employee's third and fourth fingers on his left hand. He was not hospitalized.

NarrativeAt 9:55 a.m. on September 6, 2017, Employee #1 placed his hand into the unguarded point of operation to retrieve a fallen item in the machine. Black and yellow striped hazard tape was applied around the opening and a sign posted stating to keep hands away because a piece of guarding had been removed in order to accommodate different size cartons. The employee's thumb was between the sliding metal block and the rail at the end of the block. The block slid across his thumb, scraping the thumb to the bone from the first knuckle to the tip, cutting off the very tip of his finger. The employee was not hospitalized.
