NarrativeAt 8:15 a.m. on December 5, 2017, an employee was cleaning a tank that had residual glycerine. The employee lost conscious and was killed from an oxygen deficient atmosphere.

NarrativeAt 9:07 p.m. on September 7, 2017, an employee was told to dispose of a chemical that was no longer in use. During disposal, the employee's hands came into contact with the chemical. Although he was wearing safety gloves, the chemical came into contact with the employee's skin. The employee sustained chemical burns on both hands, which he was hospitalized and received treatment. The incident investigation identified that the chemical, called Allumoshine, was previously used for cleaning and polishing and composed of Hydrofluoric Acid, Sulfuric Acid, and Phosphoric Acid.

NarrativeAt 11:10 a.m. on August 30, 2018, an employee was performing a clean out/blow down at the Line 1 rotary airlock. During the clean out/blow down the employee started and stopped the power on the rotary airlock to perform cleaning. The power to the rotary airlock was "on" during clean out/blow down of the rotary airlock. The employee sustained amputations to his fingers.
