NarrativeAt 5:40 p.m. on October 3, 2017, Employee #1 began cleaning a rotary sieve. The employee performed lockout/tagout to begin the cleaning process. When the employee realized there was more material in the machine, he exited the ladder, removed LOTO, and started up the machine, causing the machine to push the excess material out. Employee #1 turned off the machine, went back up the ladder to resume the cleaning process, forgetting to perform LOTO. He stuck his hand into the machine and his right index finger was amputated but he was not hospitalized.

NarrativeAt 10:00 a.m. on June 19, 2017, an employee was in the shop trouble shooting and adjusting a CNC milling machine (Haas VF-2D #1072943) which was not cutting correctly. The employee leaned into the machine and was checking the setup of the parts when something flew off and penetrated his chest. The employee was transported to Mission Hospital where he had surgery and was hospitalized.

NarrativeAt 11:00 p.m. on February 14, 2017, Employee #1 was packing tubes in a box from the moving plastic cleated conveyor on Line 21 in the Packaging Department. Th e employee's work glove became caught on the moving conveyor. His hand came int o contact with the static plate, severing the top of his right index finger.
