The accident investigation below was opened on Friday, October 13th 2017 by OSHA representitives in Gainesville, TX after learning of an incident at Ifs Coatings Inc. which had occured on Tuesday, October 3rd 2017. NarrativeAt 5:40 p.m. on October 3, 2017, Employee #1 began cleaning a rotary sieve. The employee performed lockout/tagout to begin the cleaning process. When the employee realized there was more material in the machine, he exited the ladder, removed LOTO, and started up the machine, causing the machine to push the excess material out. Employee #1 turned off the machine, went back up the ladder to resume the cleaning process, forgetting to perform LOTO. He stuck his hand into the machine and his right index finger was amputated but he was not hospitalized.
