NarrativeAn employee was using a sit-down powered industrial lift to retrieve a pallet that contained four 55-gallon drums. When the employee lifted the top pallet, the corner of the pallet broke, causing a 55-gallon drum to fall to the ground and strike another employee on the ground. That employee suffered a fractured right arm and hip, as well as back and neck injuries.
On Tuesday, February 3rd 2015 OSHA reps took a report of a serious accident in Fort Edward, NY after learning of an incident at Washington-Saratoga-Warren-Hamilton-Essex Boces which had occured on Friday, January 30th 2015. A 66 year old had been killed when the employee died of heat attack at work.

NarrativeAt approximately 3:00 p.m. on January 30, 2015, an employee, working on the second shift between 3:00 p.m. to 1:00 p.m., was found by the evening Shift Supervisor, on a hallway floor in a semi-conscious state. The Supervisor called an ambulance and the employee was brought to the hospital. The employees family informed the employer that the employee died on January 31, 2015 of myocardial rupture, Type A Aortic Dissection.
