NarrativeAt 5:23 p.m. on November 21st, 2018, Employee #1 was driving a forklift and bringing pallets of material and dumping the material beside a crane. During this work Employee #1 went around a corner with the forks (minus a load) raised 8 to 10 feet whereupon the forklift tipped over onto its side. Employee #1 was not wearing the seatbelt and was thrown from the seat and struck by the forklift overhead protection. Employee #1 sustained fatal head injuries and was pronounced dead at the scene.

The accident investigation below was opened on Thursday, March 16th 2017 by OSHA representitives in Greensboro, NC after learning of an incident at Ray'S Southern Auto Auction, Inc. which had occured on Tuesday, November 29th 2016. A 56 year old man working as a employee was killed on the job when the employee fell from truck ladder and later dies

NarrativeOn November 29, 2016, an employee was loading and unloading cars from a large c ar hauler. He missed a step or slipped from a step on a fixed ladder made into the frame of the car hauler and fell to ground on the concrete pavement. No one observed the accident and the employee suffered a severe herniated disk and sp inal stenosis which resulted in surgery. The employee died in the hospital afte r surgery.
