NarrativeAt approximately 8:00 a.m. on May 24, 2017, Employee #1 was troubleshooting an electric motor on a automatic garage door opener. The garage door abruptly closed and caught his vest and clothing, which became entangled within the spinning spring coil of garage door. As his clothing entangled in the spring coil, it choked him and fractured his shoulder. Someone turned off the automatic garage door and summoned emergency medical personnel. They arrived and transported Employee #1 to a medical center, where he underwent treatment on the shoulder fracture and was then hospitalized.
The accident investigation below was opened on Wednesday, October 27th 2010 by OSHA representitives in Oceanside, CA after learning of an incident at North County Transit District which had occured on Monday, August 30th 2010. A employee working as a Machinery maintenance occupation was injured on the job when the employee was struck by cover and suffered arm laceration

NarrativeAt approximately 10:20 a.m. on August 30, 2010, Employee #1 and two coworkers we re attempting to inspect the bearings of a fan. They positioned the 215 pound da mper cover of the fan into a vertical position by rotating it on its hinge axis. Due to the weight of the cover, a coworker needed to change his position and re leased the damper cover. The two remaining employees were not able to control th e weight, and before the third employee could get back into position, the cover came down on the arm of Employee #1, causing a depth laceration to his left fore arm. He was transported to a medical center where he was admitted and received t reatment for three days.
