NarrativeAt 3:45 p.m. on November 12, 2018, Employee #1 was loading a flatbed trailer with crane outriggers. As Employee #1 was attempting to strap down the outrigger, the outrigger rolled off the trailer striking Employee #1. The load was not secured. The employee was struck by the crane outrigger and was killed from head injuries.

NarrativeAt approximately 3:22 p.m. on January 3, 2018, an employee was working in warehouse. The employer provides stevedoring services, specializing in unloading and handling a variety of cargo from incoming ships. The employee was operating a Caterpillar forklift (Model No. P33000; Serial No. T39A-10210) to set up and place bins in preparation for an incoming vessel. The employees supervisor entered the shed in his truck and found the employee under the rear left tire of the forklift. The forklift the employee was operating had collided with the concrete base of a piling. There was damage to the concrete base of the piling and concrete was broken away in pieces around the base. The left fork had been bent upwards approximately 6" from the impact with the concrete base. The employee was unseated from the forklift when he collided with the concrete base of a piling, fell approximately 6 feet to concrete and was then ran over by the left rear tire of the lift and was killed. The seatbelt of the lift was tested and found to be in working order. The forklift was later inspected by a mechanic working with the employer and it was found that multiple safety devices on the lift, including the seatbelt warning light, had been disabled.
On Thursday, December 7th 2017 OSHA reps took a report of a serious accident in Shelton, WA which had occured previously at Manke Marine. A 37 year old man working as a was killed on the job when the employee was struck by mooring line in the head and was killed

NarrativeAt 4:45 a.m. on December 7, 2017, an employee had secured one mooring line for a barge that was being pushed into it's moorage at a customer's dock. The employee was struck in the head by the mooring line when it snapped and was killed.
