NarrativeAt 11:30 p.m. on February 19, 2019, an employee and coworker were working in an airport maintenance shop to replace a multi-piece tire. After removing the eleventh bolt from the rim, there was a sudden pressure release that severed the last bolt. The employee was struck in his head by the top half of the tire rim section and was killed.

NarrativeAt 2:50 p.m. on Janaury 29, 2019, an employee was working in a hangar and had opened the hangar doors enough to look outside. The doors of the hangar moved toward the center of the opening from the left and the right. The control panel for the doors is located on the center edge of the doors. As the employee was standing and looking out, it is believed that he inadvertently touched the control panel button which shut the doors. The employee was caught between the doors and is believed to have died instantly.
OSHA investigators arrived in Oklahoma City, OK on Thursday, September 20th 2018 and began an accident investigation at U.S. Department Of Transportation Federal Aviation Administration. A 54 year old Plumber had been killed when the employee died from heart attack

NarrativeAt 5:15 p.m. on September 13, 2018, an employee was completing connections for a new sewer line and had to stop work due to an irritating substance that was used at a nearby Federal Corrections Center permeating the worksite. The employee waited approximately 45 minutes before going back to the work area to collect his tools and leaving the work site. The employee drove home and experienced shortness of breath and coughing. The employee's wife drove him to the local hospital, where he was diagnosed with having a heart attack. The employee died while at the hospital from blockage of the coronary blood vessel.
