NarrativeAt 8:30 p.m. on January 10, 2018, an employee was placing hooks on the rear of a flatbed truck when he was struck by another vehicle. The employee became pinned between the rear of the flatbed, and the front of the other vehicle, resulting in his death. The cause of death was attributed to severe blunt force trauma to the chest and neck.
OSHA investigators arrived in Madera, CA on Friday, December 1st 2017 and began an accident investigation at Nick'S Towing. A 55 year old Mechanical control had been killed when the employee died of natural causes

NarrativeAt 9:15 a.m. on November 29, 2017, Employee #1 and a coworker were standing by the fire, outside of a small shop. As they proceeded to go inside of the shop, Employee #1 placed his right hand on the edge of a wooden desk and collapsed to the floor. The employee fell on his face on the concrete floor. The coworker turned him face up and yelled for help. Emergency services were called and the coworker administered chest compressions while he was on the phone with emergency services until they arrived. Employee #1 died of natural causes.

NarrativeAt 6:45 a.m. on November 28, 2017, Employee #1 was hoisting a sea train onto the bed of a trailer. The employee was standing adjacent to a low bed trailer while operating the trailer winch line controls. A 0.375 inch Campbell Grade 70 chain, which failed during the operation, struck the employee in the head. Employee #1 sustained a head laceration and a skull fracture, and was hospitalized.
