NarrativeOn December 7 2012, Employee #1 of R & H Produce Co. Inc., was discovered unresponsive in a warehouse after operating a propane powered forklift. EMS responded and Employee #1 was transported to Wake Medical Center and later to Duke University Medical Center for Hyperbaric Medicine. Employee #1 was kept overnight and received hyperbaric oxygen therapy.

NarrativeOn November 5, 2010, Employee #1 was placing a bin off a forklift onto a custome r's truck when the forklift rolled forward, pinning Employee #1 between the fork lift and the customer's vehicle. Employee #1 was taken to the hospital for his i njuries which consisted of a fracture of the lower leg. Employee #1 was hospital ized for two days.
