NarrativeAt 1:00 p.m. on October 9, 2017, an employee was trimming a tree that had several limbs hanging over a mobile home using an Echo chain saw model CS-400 and was working from a JLG telescopic boom lift. The employee climbed off the boom lift and onto the tree to cut one final section of a branch. The employee was approximately 13 feet from the ground and was struck by the cut tree section. He was knocked him from the tree and he fell to the ground. The employee was admitted to the hospital and suffered a fractured wrist, pelvis, and a laceration to his head. The employee was discharged from the hospital on October 11, 2017.

NarrativeAt 8:00 a.m. on July 31, 2017, an employee was removing old weather stripping /sealant from a roof. The employee threw a long piece of weather stripping into a trash truck that was parked below. One end of the weather stripping touched an overhead power line while the other end was still in contact with the metal roofing and the employee was electrocuted.

NarrativeAt 11:30 a. m. on July 29, 2017, Employee #1, a non-licensed contractor, was attempting to install a breaker switch to a 480-Volt electrical subpanel located at the North side of the shipping area. The electrical panel was not de-energized, and a non-insulated screwdriver was used to perform the task. The screwdriver contacted a part of the energized panel and created an electrical arc flash, causing third degree burns to the employee's upper body. The employee was working alone and there were no witnesses to the incident. Employee #1 was hospitalized for his injuries.
