NarrativeAt 1:00 p.m. on October 9, 2017, an employee was trimming a tree that had several limbs hanging over a mobile home using an Echo chain saw model CS-400 and was working from a JLG telescopic boom lift. The employee climbed off the boom lift and onto the tree to cut one final section of a branch. The employee was approximately 13 feet from the ground and was struck by the cut tree section. He was knocked him from the tree and he fell to the ground. The employee was admitted to the hospital and suffered a fractured wrist, pelvis, and a laceration to his head. The employee was discharged from the hospital on October 11, 2017.

NarrativeAt 4:00 p.m. on May 4, 2017, Employee #1, a part-time laborer, and a coworker, the onsite caretaker, were cleaning out storage units and hauling away old, unused materials. The two workers began moving plywood out of the area, with Employee #1 working on foot while the coworker operated the forklift. They had just finished moving the plywood on the far end of the lot when the coworker started to back the forklift out. Employee#1 hopped onto the forklift's left running board and the coworker, looking over his right shoulder, started backing out. As the forklift entered a narrow area, Employee #1's left foot was crushed between the forklift and a large metal shipping container, resulting in multiple fractures and injuries that eventually led to the amputation of the left leg below the calf.
