NarrativeAt 10:15 a.m. on February 28, 2019, an employee was repairing a pallet jack that was elevated by way of a 7 ton jack stand. During work, the pallet jack slid from the jack and struck the employee. The employee was crushed and killed. The incident investigation identified that there was no blocking or bracing in place.

NarrativeAt 9:45 a.m. on October 28, 2017, an employee was installing a hose reel cover and fell from a 20 foot A-frame ladder. The employee amputated his left hand's middle finger and was taken to San Jose Regional Center before being transferred to Stanford hospital. The employee underwent several surgeries to reattach the finger but none of the operations were successful.
