NarrativeAt 8:45 a.m. on November 6, 2017, an employee was filing the jaws of an operating lathe in order to make the part lay flat. The employee lost his grip on the file, which fell from his hand and contacted his stomach. As a result, the employee sustained internal bleeding and was hospitalized.

NarrativeAt 5:25 p.m. on February 10, 2016, an employee was lifting a transmission. The automobile lift failed and the rack collapsed, causing the vehicle to fall. Th e employee was crushed and killed.

NarrativeAt approximately 2:00 p.m. on June 18, 2012, an employee of Julie L. Miller dba A & A Transmission Center was working with an automotive lift to lift a GMC Denali. As he lifted the Denali, the power chain of an automotive lift failed and caused the front cross tube on the driver's side to fall and shortly thereafter causing the passenger side front tube to fall and pin the employee's right leg against the concrete floor. The employee suffered multiple fractures in the incident. The employee was transported by ambulance to Arrowhead Regional Medical Center, where he was hospitalized to treat the multiple fractures.
