NarrativeAt 10:00 a.m. on August 8, 2017, an employee was cleaning the back of the shop. As part of his job, he was lowering the hydraulic car lift. The employee caught his foot under the lift and broke the toes in his left foot.

NarrativeOn October 12, 2010, Employee #1 was operating a metal expander, when his right hand was caught in the machinery. The expander crushed the tip of his pinky fing er. He did not seek immediate medical attention, but after approximately one mon th, he underwent medical treatment. Medical treatment involved the complete surg ical removal of the entire little finger. Employee #1 was hospitalized after the finger was amputated.

NarrativeAt approximately 9:45 a.m. on April 19, 2010, Employee #1, an automotive technic ian of Shau Muffler Ogden, Inc. dba Midas Auto Service Experts, was fabricating a rack to hold used tires. He was operating a Millermatic 140 Auto-set MIG welde r in an open area behind the shop. He was welding together four used 55-gallon d rums that had contained flammable windshield wiper concentrate. While welding on the drums, they exploded, killing Employee #1.
