NarrativeOn June 8, 2018, an employee was mowing an area of a cemetery with a slope of twenty-two degree that was near a retaining wall. While near the retaining wall, the tractor became stuck. The employee dismounted the tractor and retrieved a zero turn mower from the cemetery garage to tow the stuck lawn mower. As the employee towed the stuck mower, the zero turn mower went over the edge of the retaining wall. The zero turn mower overturned and the employee received broken ribs, a punctured lung, and nerve damage to his right arm. The employee was admitted to a nearby hospital and remained there for 40 days for treatment. However, the employee's condition did not approve and he died on July 18, 2018.

NarrativeAt 8:40 a.m. on December 3, 2017, an employee was talking with a coworker who was standing at the bottom of the staircase. The employee fell to his right and over the stair rail, falling approximately 8 feet before landing on the ground. The employee was hospitalized with multiple internal injuries and broken bones. The employee died 5 days later.

NarrativeAt 3:00 p.m. on October 18, 2017, an employee was riding in a golf cart and had her legs on its dash. The golf cart struck a tent stake and threw the employee out of the cab. The employee fractured her left leg and was taken to the hospital for treatment and was hospitalized.
