NarrativeAt 4:53 p.m. on September 20, 2017, Employee #1 was in the basement garage of the employer's building. A hybrid car was parked near the employee's car and she was curious to examine the cord plugged into the wall. While walking towards the car, Employee #1 slipped, fell, and sustained a fractured right hip that required surgery. Employee #1 was hospitalized for her injury. It was noted that there was a slippery substance on the garage floor.

NarrativeAt 4:30 p.m. on June 23, 2017, Employee #1 was changing out his stove's butane canister when the new canister burst open and ignited. Employee #1 and Employee #2 was working by the stove were caught in the flames and burnt their face, hands, and forearms. Employee #3 came to their aid and also burnt his hands and forearms. The fire department were transported to the Santa Catalina Island Hospital were treated for their injuries and transferred to UC Irvine on the mainland.
