NarrativeAt 4:00 on January 16, 2018, an employee entered the GED "Butterfly" machine to clean the rollers and the machine cycled, crushing him. The employee was extracted from the machine by local fire department and was taken to Cook County Hospital where he was hospitalized with internal injuries.

NarrativeAt 1:15 p.m. on October 4, 2017, an employee was operating a table saw while cutting a wooden log and amputated a finger when it touched the blade. The initial report did not identify which finger had been amputated. Hospitalization was not required.

NarrativeAt approximately 7:00 a.m. on July 14, 2017, Employee #1 was using a machine to punch holes into a metal frame. As the punch die came down, one of his finger was in its way. The punch die struck his right middle finger and partially amputated its tip. The metal punch press lacked machine guarding. Employee #1 received first aid treatment and was not hospitalized.
