NarrativeAt 3:00 p.m. on July 7, 2018, Employee #1 completed his work shift and left the worksite in his personal vehicle. Employee #1 was responsible for the overall housekeeping of the lumber yard and to assist other employees with the staging of materials and finished product. He was discovered unresponsive in his personal vehicle offsite hours later by a passerby. The cause of death was listed as: Hyperthermia due to high environmental temperature hours. Employee #1 was working in direct sunlight under heat wave conditions for two consecutive days.

NarrativeAt 7:30 a.m. on November 16, 2017, an employee was operating the Delta DJ-20 Jointer, Model # 37-350, Serial # JOO 8275, located on the production floor. The employee was using the jointer to straighten the wooden board and hit a knot in the wood. The wood pulled the employee's hand into the jointer's blades and amputated her left hand's middle finger. Hospitalization was not required.

NarrativeAt 10:45 p.m. on August 29, 2017, Employee #1 was cleaning a machine with a moving saw blade, using an air gun. The employee reached behind the moving saw blade, and the blade grabbed his sleeve, pulling his hand into the side of the moving blade. Employee #1 received a laceration to the back of his hand. He was hospitalized for his injury.
