NarrativeAt approximately 2:40 p.m., on March 16, 2011 Employee #1 was seriously injured when he was operating an automatic metal foil shear press. Just before the accident, the metal foil got bubbled up on the table and it would not go through the cutting blade. As Employee #1 pushed the metal foil with his left hand, his middle finger got caught between the hold-down device and the table causing a crushing injury and his finger had to be amputated at the hospital.
