Primary Aluminum Production

NarrativeAt 6:45 p.m. on October 30, Employee #1 was standing by the staircase of Blanking Machine #1. At the same time, a forklift operator was attempting to load a 160 pound roll of plastic film onto the Blanking Machine. The roll tilted and fell approximately six feet onto the employee's right hand that was resting on the stair railing of the Blanking Machine, amputating the tips of his middle and ring fingers of his right hand.

NarrativeAn employee was walking next to the location where a carbon anode got stuck on the end of a conveyor. The anode dislodged and fell onto the passing employee's right foot fracturing it.

NarrativeThe employee was working at the stacker end of the cut to length line. The table in the stacker was being raised to be at the same elevation as the conveyor on the cut to length line so product could begin to be stacked. The employee went to remove a board from the table and his right middle finger was caught between the board on the table and the stacker resulting in a fingertip amputation.
