NarrativeAt 8:00 p.m. on October 7, 2017, an employee noticed that the grinder return bin had become clogged. The employee grabbed a broom handle that was kept in the area and pushed it inside the grinder return bin in an attempt to unclog the equipment. The employee stated that there was a noise, and then realized that the slide gate had closed while the hand was inside the grinder return bin, resulting in amputation of his hand.
The accident investigation below was opened on Thursday, August 17th 2017 by OSHA representitives in Helena, AL which had occured previously at Abc Polymer Industries, Llc. A 47 year old Industrial machinery repairer had been killed when the employee was dragged into a godet/roll machine and was crushed

NarrativeAt 6:45 p.m. on August 16, 2017, an employee was cutting a wrap from a godet/roll machine, Fare S.P.A. model number 354 R85 174, date 8 1989, that was running. The employee's body was pulled into the machine and was crushed when caught between the webbing and rollers.

NarrativeAn employee was monitoring the production conveyor to remove defective products from production. While he was pulling a large piece of plastic off the conveyor, the piece of plastic fell into the equipment. The plastic started making a loud noise, so the employee attempted to remove it. His hand was then pulled into the equipment's wheel area. His right thumb and index finger had to be partially amputated. The guard was in place at the time.
