OSHA investigators arrived in Bolivar, OH on Wednesday, December 20th 2017 and began an accident investigation after learning of an incident at Primary Packaging, Inc. which had occured on Monday, December 4th 2017. A 21 year old man working as a Machine operator was injured on the job when the employee's fingered are injured in printing press

NarrativeAt 8:00 a.m. on December 4, 2017, an employee had a rag in hand and was inspecting a Wolverine printing press after its barrier guard was removed from the ingoing nip point between the print cylinder and anilox roller. As he was holding a rag in hand and inspecting the press, the rag became caught in the ingoing nip point of the two. Both the rag and the employee's hand were pulled into the machine. The employee's middle and index fingers on his right hand were degloved, which his middle finger later required amputation at its second knuckle.
