NarrativeAt 9:30 a.m. on December 5, 2017, Employee #1 was cutting concrete with a Stihl TS700 saw. The blade broke, sending three pieces airborne. A piece of blade struck the right foot of Employee #1, causing lacerations.

NarrativeAt 8:00 a.m. on November 27, 2017, Employee #1 was working in the Assembly Department, operating a one ton Dayton Overhead Crane to lower a cement cover. The hook on the overhead crane did not have a safety latch. As Employee #1 was using his right hand to operate the overhead crane and his left hand to guide the cement cover, the cover slipped off the hook and landed on his left hand. His left thumb was amputated as a result.
