NarrativeAt 12:00 midnight on November 6, 2016, an employee was walking into a facility when he was hit by a forklift. The employee was admitted to the hospital for 4 days and released. While at home, the employee was not feeling well and returned to the hospital where he passed away from a blood clot.

NarrativeAt 10:40 a.m. on October 6, 2016, Employee #1 was standing on the production floor talking to another employee. When the other employee had left, and then turned around, he saw that the employee was on the ground. Cause of death was not reported.

NarrativeA lead person was adjusting the edge grinder. An employee walked to the back of the machine and placed his hand on the end of an adjustment rod sticking out of a pneumatic cylinder. The lead did not realize the employee's hand was on the rod, so he actuated the machine to test his adjustments. The employee's left index finger was then pinched and amputated between the cylinder housing and the rod end stop.
