NarrativeAt 11:00 a.m. on February 27, 2019, an employee was inside a shipping container that contained crates filled with glassware. While inside the container, several of the crates fell over and was crushed the employee. The employee was killed.

NarrativeAt 1:15 p.m. on February 12, 2018, an employee was clearing a clog in the air rotary and the paddles began to rotate. As they rotated, the employee's right ring finger was pinched and the tip of his finger was amputated. The employee was hospitalized.

NarrativeAt 1:00 p.m. on November 2, 2017, an employee was trying to troubleshoot a plumb bob for a silo. The employee went around the interlocked gate into the distributor arm area and the distributor arm moved and eventually struck the back of the employee. He was pinned between the mouth of the arm and the opening of the silo. The employee was crushed and fatally injured.
