NarrativeAt 12:15 p.m. on November 17, 2016, an employee had completed making adjustments to the conveyor belt that was part of the Bekum Plastic Molder. The employee opened the gate guard on the left side of the machine and had his head crushed when the machine cycled.

NarrativeOn or about 5/12/2016, an employee received a partial fingertip amputation injury while operating a paint brush hand trimmer machine, known as a hand sharp edge. While manually holding a paint brush in the machine, her finger slipped and contacted the rotating blade of the machine. The contact resulted in a partial fingertip amputation.
