NarrativeOn August 23, 2017, an employee had just finished a product run and was in the process of setting up the next job where the employee needed to add a part to the sewing machine. The employee had not locked the sewing machine and, while checking the belt tightness, a coworker hit the activation lever, causing the machine to cycle. The employee amputated his right index finger when the machine cycled itself. The employee was hospitalized.

NarrativeAt 4:25 p.m. on June 30, 2017, Employee #1 was attempting to take the lid off of the conveyor to clean inside. As the employee lifted the metal lid, which was approximately two feet wide and five feet long, the lid caught on an adjacent lid, causing it to pinch his finger between the sharp edges. The employee was not wearing gloves, and his right pinky finger was amputated above top knuckle.

NarrativeAt 3:30 a.m. on June 5, 2017, an employee was operating machines in the TWP room. He left the room to perform a visual inspection on the bag house filter bin. While attempting to reseat the flexible skirt, he slipped on the edge of the bag house platform and contacted an unguarded rotary valve with his right hand. As a result, his right thumb and index finger was amputated.
