NarrativeAt 10:30 a.m. on December 3, 2017, a 47 year-old industrial/maintenance mechanic was repairing a Sudmo 2-way water valve (Model: D650-E-30, XV3135) in a brew house. The valve was located approximately 30 feet in the air around piping and a tank. The employee accessed the valve using an aerial lift. The employee requested a coworker to operate the pneumatics for the valve from the control panel so that the valve could be removed from the valve seat. The control room was located on an elevated platform within eyesight of the employee. After the valve was removed from the line, another valve (XV1341), which had previously been malfunctioning, opened up. The computer program, called for water. Water from the hot water tank (approximately 160-165 degrees F) sprayed on the employee. The employee sustained 2nd and 3rd degree burns and was hospitalized. The operator attempted to stop the pump from the control panel but was not able to find the correct pump.
The accident investigation below was opened on Monday, November 20th 2017 by OSHA representitives in Columbus, OH at Ardent Mills, Llc. A 45 year old man working as a Supervisor was injured on the job when the employee's fingered are amputated when caught in rollers

NarrativeAt 10:30 p.m. on November 10, 2017, Employee #1 was reaching in and attempting to clear an object in the rollers of the Buhler-Miag 4 Midds #1 roller mill/roll stand with his right hand. The rollers pulled his hand into the roller mill/roll stand and amputated the right index finger at the second joint, the right middle finger at the first joint, and the fingertip of the right ring finger.

NarrativeOn September 29, 2017, the employee had been informed about a leaking hydrous valve and drove a 4-wheeler to the site. After arriving at the site, the employee was overcome by the chemical smell and left the area. While driving away, the employee wrecked the 4-wheeler, suffering a broken collarbone, wrist, and facial injuries. The employee was taken to a hospital where he was admitted overnight.
