NarrativeAt 9:30 p.m. on July 6, 2017, an employee was working on the Adamatic ADR-2 dough machine. The employee reached into the hopper to remove some jammed dough while the machine was turned on and the employee's fingers made contact with the moving blades located at the bottom of the hopper. The employee amputated their middle and ring fingers.

The employee slid their hand into the press when a presence sensing device was not correctly adjusted and it was trapped within the machine.
