NarrativeAt 9:58 a.m. on November 3, 2017, an employee had set the steam to 128 degrees Fahrenheit to help remove product that stuck to the interior walls of the tank. The water reached a temperature of 212 degrees Fahrenheit, and had begun to splash out of the upper lid of the tank. The employee opened the lip and was splashed on his left leg, hand, and foot with scalding water. The employee sustained second degree burns and was hospitalized.

NarrativeAt 4:00 p.m. on September 11, 2017, an employee was working for a manufacturer of dry pasta, dough, and flour mixes from purchased flour. He was cleaning residual dust from a wall located behind a dust collector. He was less than 3.0 meters (10 feet) above the ground. He was ascending the ladder to tie off. The ladder slipped underneath him, and the employee fell. He sustained a broken tibia. He was hospitalized.

NarrativeAt approximately 3:45 p.m. on December 5, 2016, an employee was starting up an oven and band for snack foods production. The employee was discovered unresponsive between the roller and band. The employee appeared to be pulled into ingoing nip point and killed. No guarding for the ingoing nip point was in use at the time of the incident. The employee's discovered position was with his right upper arm between the roller and the band, and his left arm between the roller and band at their elbow.
