NarrativeAt 5:00 a.m. on September 27, 2018, the employee was unloading and washing a truck tanker with water that had contained poultry blood. The employee was found unresponsive on top of the tanker with his head over the tanker opening. The employee was killed from exposure to ammonia and hydrogen sulfide vapors from the tanker truck.

NarrativeAt 5:55 a.m. on October 19, 2017, an employee was working on the drive motor for the paddles in the DAF platform. The torque from the motor caused the gearbox leg mount to break and the motor was putting pressure on the top of the mounting chain. The employee pulled the chain from the top and as the motor came down, it caught the employee's middle finger between the chain and sprocket. The employee suffered an amputation. Hospitalization was not required.

NarrativeAt 9:30 a.m. on September 29, 2017, an employee was working in the gyro room assisting the gyro spit "trimmer" by clearing meat scraps from near the vertical band saw. The employee swept some scrap meat from the band saw table using his right hand and then began using his left hand with the right hand to sweep the scrap meat towards him. The employee placed his left hand too close to the vertical saw blade and cut off a portion of the tip of his left index finger. The employee was hospitalized and treated for his injuries.
