NarrativeAt 12:00 p.m. on October 4, 2017, Employee #1 was plasma spraying a coating inside of a 10 foot long by 7.5 inch pipe. The employee was not wearing respiratory protection and inhaled nickel-chromium fumes. Employee #1 developed acute respiratory failure and was hospitalized until November 9, 2017, when he died from a fatal cardiac event.

NarrativeAt 5:30 p.m. on September 18, 2017, Employee #1 was performing cleaning operations inside of a kettle that housed a bath of molten zinc at 850 degrees Fahrenheit. His left foot fell into the molten zinc and he suffered second degree burns to his left lower leg and foot. Employee #1 was hospitalized for his burns.

NarrativeOn August 11, 2017, an employee was hand feeding aluminum wire into a pipe threading machine to curl it around a pipe to create a curled handing holder for parts to be anodized. The employee's left hand glove was caught by the machine and pulled his hand into the nip point where he suffered four finger amputations. The employee was taken to the hospital and had one finger reattached. The employee was also admitted to the hospital.
