NarrativeAt 11:45 p.m. on January 18, 2018, an employee was operating abrasive blasting equipment to prepare industrial rollers for plating. The employee was working alone. For an unknown reason, the employee became entangled with the equipment at its coupler location and was killed. The employee was later found with his shirt and upper portion of his torso wrapped around the coupler and shaft (PTO and journal).
On Thursday, November 30th 2017 OSHA reps took a report of a serious accident in Canton, OH at Olymco, Inc.. A 39 year old man working as a Fabricating machine operator was injured on the job when the employee suffered finger amputation on polishing wheel

NarrativeAt 9:00 a.m. on November 27, 2017, Employee #1 was polishing an exhaust tip when the part became caught up in the polishing wheel. The wheel pulled the part out of his hands, causing his left hand to contact the sharp part of the wheel. Employee #1 sustained an amputation of one of his fingertips on his left hand.

NarrativeAt 9:45 p.m. on October 10, 2017, an employee was dumping parts into a barrel as part of a drying process, which he had filled several barrels prior. While getting ready for the next barrel to be positioned to receive parts, the employee depressed a button for a hoist to retrieve another barrel. Noticing that the hook on the barrel had not been removed, the employee approached the barrel, placing his body in front of the hoist. The employee as struck and killed by the hoist.
