NarrativeAt 10:10 a.m. on November 11, 2011, Employee #1, a machine operator for Teledyne Reynolds, Incorporated, tried to pull a pellet (product) out of the mold preform machine with his hand, instead of with an extension tool. He suffered multiple finger amputations of the right hand. Employee #1 was transported to UCLA Medical Center in Santa Monica, where he was hospitalized and underwent surgery. The machine appeared to have been in the automatic mode. Cal/OSHA determined that the incident was caused by the employer failing to ensure that this machine was properly guarded and for failing to ensure that an extension tool was used for this task. Lockout/Tagout procedures had not been developed for this machine nor had affected employees been trained.

NarrativeAt approximately 9:50 a.m. on December 2, 2009, Employee #1, a production superv isor, was attempting to repair the screen of a 220V Everspring printing equipmen t screening machine, Serial Number 0505001. This machine builds up ceramic capac itor insulation using a screening process that adds the material a single layer at a time. The material is laid down on the plate bed of the machine by an alumi num-framed screen measuring approximately 27-in. square that is operated by a pn eumatic piston system. The operator reported to Employee #1 that a tear had deve loped in the screen. Employee #1 attempted to repair the screen with a piece of tape. With the machine still energized and the screen in the highest position, 1 5-in. above the plate bed, she leaned over the controls at the front of the mach ine to access the under side of the screen. She contacted the switch that contro ls the vertical movement of the screen causing it to lower, pinning her in the m achine. Employee #1 was hospitalized with multiple fractures.
