NarrativeAt 7:45 a.m. on February 15, 2018, an employee was looking for a client. She slipped on wet grass and fell. The employee fractured her right ankle and dislocated her elbow.
OSHA investigators arrived in Los Angeles, CA on Tuesday, September 12th 2017 and began an accident investigation which had occured previously at City Of Los Angeles Department Of Recreation And Parks. A 39 year old Recreation worker had been injured when the employee collapsed and was hospitalized

NarrativeAt 12:00 p.m. on September 8, 2017, an employee was working with children as a recreation assistant.The employee was sitting under the shade in the lunch area of the yard towards the back of the facility, when he collapsed and landed on his right side. The employee was unresponsive, which emergency services were contacted and, upon arrival, transported him to a nearby. He was hospitalized and treated for an undisclosed medical condition.

NarrativeAt 8:15 a.m. on July 21.2017, an employee was providing care to a resident patient with autism. The resident lashed out, striking the employee repeatedly in the head and face. The employee suffered headaches, blurred vision, nausea, and began vomiting.
