NarrativeAt 2:00 p.m. on November 7, 2017, an employee was working for a painting company, assigned to perform painting at a residence. The employee was assigned to paint the eve of the residence front porch. Using an 8 foot stepladder, which was resting on the top of the pitched roof, the employee accessed the eve. During work, the employee slipped and fell approximately 20 feet to a concrete patio. The employee was not wearing fall protection. The employee sustained a compound fracture, resulting in hospitalization for treatment.
OSHA investigators arrived in Greenwich, CT on Wednesday, November 1st 2017 and began an accident investigation at W. R. Berkley Corporation. A 29 year old Accountant had been injured when the employee was hospitalized after exposure to carbon monoxide

NarrativeAt 5:09 p.m. on October 24, 2017, an employee was working at an insurance agency. She and her coworker were working in an office building. During the day, some workers began to complain of feeling ill. Exhaust gases from the boiler were entering the fresh air intake to the building. The exhaust gases contained carbon monoxide. The employee was exposed to the carbon monoxide and she was hospitalized.
