NarrativeAt 2:00 p.m. on December 7, 2017, an employee was assisting several horse owners and trainers with releasing horses from their stalls during a fire. One horse ran back into the stable and the employee went into the stable to lead him out when he noticed a large group of horses running towards him. The employee was struck by one of the horses and trampled by several others. Coworkers found the employee on the ground, and the employee was transported to the local hospital by ambulance. The employee was treated for multiple rib fractures, facial trauma, lacerations, and contusions. The employee was hospitalized.
OSHA investigators arrived in Santa Monica, CA on Wednesday, September 20th 2017 and began an accident investigation after learning of an incident at Doug O'Neill Racing Stable, Inc. which had occured on Saturday, August 5th 2017. NarrativeAt 7:00 a.m. on August 5, 2017, Employee #1 was assigned to jog horses for the Assistant Trainer. After jogging the stallion, the horse was returned to the 12 by 12 foot cinder block wall stall. The metal ring on the bottom of the horses halter was clipped using a 2.5 foot bungee cord with self-closing spring loaded clips, and then clipped to a metal hook at the center back of the stall. At the entrance of the stall, there was a barrier chain at the front opening. The employee had secured the stallion to the bungee cord on the wall, and then removed the lead line and stud chain. The horse was tied so that when employees had to exit or enter the stall, they were exposed to the rear of the horse. While exiting the stall to duck under the chain barrier, the horse kicked the employee, causing numerous facial fractures. Employee #1 was transported to a nearby hospital, but then transferred to a larger facility, where she was hospitalized for seven days.

NarrativeAt approximately 10:00 a.m. on August 4, 2017, an employee was riding a horse. The employee fell from the horse to the ground when the horse became startled. The employee suffered fractured vertebrae and was hospitalized.
