NarrativeAt 11:00 a.m. on December 28, 2017, an employee was inspecting mushrooms on a rack approximately 7.5 feet above ground. The employee fell to the ground, striking her head on the concrete floor. The employee was killed onsite.

NarrativeAt 12:30 p.m. on December 11, 2017, Employee #1 was adjusting the Omori wrapper to the new till size for the next production run. The employee left the guard up, and the safety interlock did not function. When the employee turned on the machine, it pulled the employee's finger into the cutting point. Employee #1's right index fingertip was amputated by the mechanical blade on the Omori wrapper.

NarrativeAt 9:30 a.m. on July 1, 2017, an employee was working in the Eryngii area conducting washroom operations using a pressure washer and ladder, while cleaning off spores from the cooling unit near the exhaust fan. The employee was standing on the third rung of a 12 foot Werner ladder and fell from it. The employee suffered a fractured left hand and sustained several fracture on the left side of his face. The employee was hospitalized.
