NarrativeAt 5:42 p.m. on May 23, 2019, an employee at a golf resort was driving a maintenance cart across the street when he was struck by a vehicle. The employee was life flighted to a hospital, where he later died.
OSHA investigators arrived in Chicago, IL on Monday, September 10th 2018 and began an accident investigation after learning of an incident at Kimpton Hotel & Restaurant Group, Llc which had occured on Sunday, September 9th 2018. A 56 year old Electrician had been killed when the employee was killed during electrical work for coffee maker

NarrativeAt 6:30 p.m. on September 9, 2018, an employee was adding an extra electrical line for a coffee maker on a 208 volt electrical panel. The employee was found face down on top of the electrical wiring by a coworker. The Medical Examiner had not determined the cause of death at the time of this report. The employee was found by another employee on the floor, face down on top of the electrical wiring.
The accident investigation below was opened on Wednesday, January 10th 2018 by OSHA representitives in Sioux Falls, SD which had occured previously at Main & Main, Llc. A 44 year old woman working as a Housekeeper was killed on the job when the employee suffered anaphylaxis and was killed

NarrativeAt 8:00 a.m. on Januray 10, 2018, Employee #1 was cleaning a room when he was stung by a bee. He experienced a severe reaction that constricted his airways. The employee suffered anaphylaxis and died.
