NarrativeOn Just 6, 2018, an employee was operating a Crown RM6025-45 Rider Reach stand-up forklift and went to turn from one aisle into the next. The employee crashed into the shelving and was struck by the products stacked on the shelf. The medical examiner's final diagnosis was that the employee was killed from asphyxia due to the compression of the neck.

NarrativeAt 7:00 a.m. on November 17, 2017, an employee was cutting the tops of 55 gallon drums with a plasma cutter for storing scrap metal. NarrativeAt around 5:00 p.m. on September 22, 2017, an employee was working for a dairy packaging manufacturing wholesalers company. The employee was operating meat slicer. He was working alone. The slicer was a GROTE Model Number 613 VS2 slicer with Serial Number 175923. The employee was standing on a platform roughly 1.2 meters (4 feet) off the ground. The platform had the proper guardrails. The top opening of the slicer was roughly 1.2 meters (48 inches) above the platform where the employee stood. The blades of the slicer were roughly 500 millimeters (20 inches) below the top of the opening of the slicer. While the slicer was still running, the employee reached into the slicer to pull out a plastic ring. He sustained the amputation of the middle finger on his right hand to the knuckle. He further sustained the laceration of the index finger on that hand. The opening required 30 stiches. He was hospitalized.
